School Immunizations for 6th Graders

Tarrant County Public Health will be conducting a vaccination clinic to get your student(s) up to date for the upcoming school year.

Tuesday, April 2, 2024 at 9:30am

Location: Icenhower Intermediate

Fee: $10 per dose of vaccine

Vaccines needed for 7th grade will be given.

TDaP, Meningococcal (MCV) and HPV (optional) vaccines will be provided.

CONSENTS DUE TO THE NURSE BY THURSDAY, MARCH 28th

Parents are not required to attend this event. Consent forms are available outside of clinic. Please FULLY complete a consent form for each vaccine to be given to ensure that your child receives all of the vaccines you request. The following items must be included:

1. Name and date of birth of child and parent

2. Full address of child

3. Parent signature for consent of each vaccine to be given

4. All medical questions answered on consent form

5. Payment: Staple or paperclip money or check made out to TCPH OR write Medicaid number on each consent form. Attach a copy of BOTH SIDES of Medicaid or CHIP card to the consent packet.

**Make sure your child eats breakfast the morning of April 2nd.
